Community Support
Through our Community Support Service, whether it’s delivering a short or long-term community care package, placement breakdown support, or providing transition care and support from a hospital to a community setting or from children to adult services, we support and empower adults and children with diverse and complex care needs to live life to the full and as independently as possible.
Supported Living
Through our Supported Living Service we aim to help people achieve greater independence and lead rich and fulfilling lives. Supporting adults and children with a range of support needs including learning disabilities, autism, physical disabilities, acquired brain injuries and mental health. Helping them access the opportunities and activities open to everyone in their community.
